List of Requirements
- Letter of Intent / Application Letter:
- Fully accomplished Personal Data Sheet (PDS) with a recent passport-sized picture (CS Form No. 212, Revised 2017), which can be downloaded at www.csc.gov.ph;
- Photocopy of certificate of eligibility/rating/license (Preferably but not required);
- Photocopy of Diploma and Transcript of Records. (if any) Interested and qualified applicants should signify their interest in writing.
